We will accept assignment and be paid directly by your insurance company. At your first visit, you will pay only what your insurance company will not cover. Payments for subsequent visits will be discussed with you at your first visit. Please bring any special insurance forms, your insurance card and driver's license with you at the time of your visit. We participate in many area health plans. If you belong to an HMO, we will need a referral fro your primary care physician.
